What does this sound like ?

I am a 21 year old female. I used to be anorexic and bad a pretty bad exercise addiction - running around 20km daily. Now running about 7 km a day and have lately been feeling like there is some sort of strange feeling in my knees, hard to describe, same creakiness as you would expect with the worn out door hinges?

I don't have any major pain in knees after running, just a constant dull ache thats worse with squatting and going up and down stirs - but if I place my hand on the knee cap and then bend my knee and straighten out , I can feel the vibrations like how a door hinge gives a vibration as we move the door. Really concerned. Please help!

I can feel something like a weird grinding/vibrating sensation "in" my knee (Not in the same way I imagine tendonitis to do) when I move my feet up and down after having them placed on the ground (especially noticeable when driving), or even sometimes lying down flat on my back rotating my leg while just having my heel on the ground. I also can push at my patella with my fingers and create the same creaking sensation.

Also I sometimes get some redness and inflammation around my knee in the afternoon? Please help! very scared this is osteoarthritis or chondromalacia. I ave had X-Rays and it does not show anything

Sorry to hear about the discomfort. Yeah, x-rays won't show cartilage damage effectively, but they can show evidence of patellar tilt, or maltracking to the trained eye. You'd need a CT or MRI to really see if there's cartilage damage.

Regarding the injury itself, there's a lot of things it could be, to the point where it likely isn't valuable to list them all until you get more info. The fact that you're still running daily is something that you might consider taking a break from and see how your knee improves from that alone. I've done many a marathon in my younger days, and 20K a day is really not something that too many people can do for long without eventually developing some knee pain.

Agree with Dave33, this is most likely chondromalacia patella, grade 2 or 3. Get an MRI, it should confirm it. X-rays would not detect it. If you also have inflammation you may also have some impingement. Get an MRI. Chondromalacia is generally more prevalent in women.

I am a former runner. It could be a solo pursuit or one can have a community, have done both. You run and run, it feels good, you think it's healthy, you keep running. In my hey days used to say "I don't put my shoes on for less than 20km". Weekend runs would be 30k-50k. But all the while you are doing a number on your cartilage, and few, if anyone, would tell you this. Moderation is key - I think a 5k mid-week run and 10k weekend run is all one needs, when supplemented with other low-impact sports. More than this and you are asking for trouble.

On the plus side, at 21 you have a good chance at recovery. I'd stop running for at least two years, substitute with walking, moderate cycling, swimming, yoga. If you do recover you can resume running but don't push it.

Can you heal chondromalacia patella ? I’m really scared, I want to run for the rest of my life and this pain is stopping me from even walking up the stairs!

Have you had scans to confirm the issue?  Have you ruled out other overuse issues such as patellar tendinitis, potential meniscus tear, ligament inflammation or tears etc?
See a knee specialist and get thoroughly examined :)
Chondromalacia patella is softening of the cartilage on the back of the kneecap, a symptom with another cause be it wear and tear, chronic inflammation, overuse.

Agree with what others have said. You need an MRI to work out what the actual issue is. From there, you'll be in a position to take advice from qualified professionals.

Thank you for all your advice, however it is really hard for me to get access to medical care as I still live with my parents without a car and they refuse to take me to any appointments because they just chalk up all my ailments to byproducts of my anorexia and they dont really care.

Is there perhaps a friend who can help you out with lifts? Presumably you’re in the US away from the big cities so public transport isn’t an option?
Even if the issues do stem from your running or even anorexia, surely your parents would still want you to get treatment?
If you want a diagnosis and treatment plan, you really need to be examined, a knee specialist ideally, perhaps a musculoskeletal physical therapist
