Kneecap Catching Post ACL Surgery

My son had ACL surgery 8 weeks ago with a hamstring graft. He is a 21 year old college wrestler and the team OS did the surgery.  At his post op check up his kneecap caught and the OS said that is normal. Fast forward 8 weeks and it is still happening.  He is walking with a limp because straightening his leg causes the catching.  He is being told that this can happen due to muscle imbalance, but I feel like it is causing a regression in his rehab.  He is at rehab daily and working out daily on his own, so it is not due to lack of prescribed rehab. I am worried about the limp and that it will cause additional problems.  My daughter is a college soccer player and also had ACL surgery with quad graft 2 years ago and had no post op issues.  Any suggestions as to next step would be appreciated.

8 weeks is still very soon after a big knee surgery, that has a good 6-12 month rehab.

Swelling in the knee can cause issues, so he should work on that as well as the muscles, the hamstring particularly will need extra time to recover.

Good that he is following his surgeon and PTs instructions to the letter, they are the experts.

Everyone recovers differently, you can’t compare your son to your daughter, especially as they actually had a different type of graft in the surgery.

He will see team trainer in 2 weeks and continue PT at home until then. So, just continue with ice, compression and quad strengthening, until then?  Thanks!

Is he not seeing a physiotherapist regularly?

He is seeing a physio 2-3X/week in person.  Team trainer remote and will be with her daily in 2 weeks.  She provides daily rehab exercises and general S & C as well.  Neither the PT or trainer are too concerned so I probably shouldn't be either.

He’s still at a very early stage post op but also coming into a very vulnerable stage as the graft vascularises. He needs to be very cautious, to avoid damaging the repair. It’s a marathon of a recovery not a sprint.

I had an ortho recommend an XRay and possible 2nd opinion to check out graft and tunnel placement.  Any thoughts on that?

I’d wait until the 6 month mark as it’s so soon after surgery, but up to you.

Why would that ortho not do the checks themselves? Aren’t they a knee specialist?
