Hemarthrosis after lateral meniscus correction
« on: January 05, 2020, 06:55:40 PM »
Question, as I can not find any data to support a decision.  Should a 2-week postop Hemarthrosis be drained if pain and impeding flexion of the knee?

43 yo lateral  bucket handle tear.  Discoid meniscus.  Cartilage and ligaments reported as “really good shape”. Attempted repair, but repair would not take and decision to repair was aborted.  Meniscus was trimmed.  Four hours postop knee swelled, and flexion went from 45 degrees to 15 degrees.  About 30cc of blood drained, blood was coagulating in the syringe, could not remove anymore. Cause of bleed believed to be either geniculate artery bleed vs synovial.

Now 2 weeks out, 70 degrees flexion and blood still present in knee (confirmed by ultrasound), difficulty ambulating without crutches.

Washout or wait?

What does your surgeon recommend?
Came off bike onto concrete 9/9/09
LK arthroscopy 8/2/10
2nd scope on 16/12/10
RK arthroscopy on 5/2/15
Lateral meniscus trim, excision of hoffa's fat pad, chondral stabilisation
LK scope 10.1.19 medial menisectomy, trochlea microfracture, general tidy up

Thank you for the response, and good question.  Surgeon is indifferent, but the hemarthrosis appears to have made the surgeon nervous about doing anything.  Very reputable individual. The indifference is why I’m asking.

A friend of mine developed arthrofibrosis following a large post op bleed, so certainly worth discussing concerns with surgeon (who likely has limited experience of arthrofibrosis as it’s quite a rare complication). Especially benefit and importantly risk of any further surgery (every surgery causes scar tissue)
