Hi All

i was an avid runner ( >100k / week ) but i had a bad fall end of March straight onto both knee caps onto tiled floor ( didn't notice a step ). Both patella took the brunt, i didnt get hands down either to break fall.

It was sore but i was able to walk ok. there some mild swelling next day but nothing serious and i could extend / move leg no issue.

i was in middle of marathon training so 3 days later, i went for run. My right knee / leg didn't seem to be moving properly but i kept running to try to run it off. Next day i had pains in hip / quads, but nothing serious. rested 2 more days and tried to run again but quads / hips sore.

visited physio who recommended i get xray to check if patella fractured, it was clear. Physio recommended to do core exercises (planks) as said was little mis-alligned to to re-intro running slowly, which i done and gradually built up mileage, all was going good.

approx 1 month later, i started getting feeling in right knee again, like a rubbing / discomfort underneath knee cap. No pain but something didn't feel right. it's kinda hard to describe.

visited physio again who said probably quads tight and done dry needling on trigger points to release. no major improvement. i can still run but don't feel as effective as did month ago.

so am not sure next step, have physio visit for later in week, but just wondering if anyone here had any thoughts?

my main worry is i tried to go back running too soon and maybe have done more damage, possibly permanent but not sure. I'm not in pain but can "feel" like something rubbing under knee cap.

Reading your post, I think that you really need to see an Orthopaedic Consultant - perhaps a MRI is required - but dont just leave it

Hi, tks for response.

No, not planning to leave it for sure. i'll see what physio thinks this week but am thinking myself i need to see medical specialist now aswell and maybe some scans.

Main worry is something permanent, or is the fact I've no pain and can still run a good sign?

I am in full agreement with John get it checked out by an OS and ask for an MRI scan, it always tempting to carry on running through the knee pain and discomfort if you are a knee long distance runner. I did the same as you, I only ran 27 miles per week doing a 9 mile circuit in just over an hour 3 times a week I felt anterior pain and discomfort while running thinking nothing of if as it must be normal to get some pain if you have been running for 9 miles. I also had a fall before my 1st scope tripping up on a loose shoelace, I carried on running after the fall that might have been a costly mistake ending my long distance running a few years later on.

Maltracking problems of the knee and PFS are common knee problems that many long distance runners experience. Jumpers knee is also common if you run on a hard surface putting strain onto the patella tendon causing inflammation of the patella tendon. I have a chronic form of patella tendonitis, have already been through 2 surgeries with a 3rd scheduled in the autumn.

Listen to your knee if it hurting when running it's best to stop and get it checked over thoroughly! We are both guilty ignoring the knee pain. I have not been able to run since January 2013, my OS said it was a pre existing condition, made it worse running when it was painful.

What I have found helpful is seeing a sports medicine doctor and sports physiotherapist before I booked in to see my OS again for my 3rd surgery. They will try none invasive form of treatment options before going ahead with surgery that the job of an OS when a referral has been made through your GP. PT or sports med doc after all none invasive options have been exhausted. I waited 3 years before I went for my surgery, my OS likes to play it safe with surgery as the very last option unless there is a significant finding on the MRi scan where surgery will be beneficial with a positive MRI scan result.

unfortunately I'm a clumsy runner and my knees have scars to prove it but in general in the past, i could usually run through a fall or knock on the knee. i've done the same on ankles and come off the bikes.

just a bit different this time but am going to get it checked properly for sure. just hope i can get back and that's a worry in back of my mind.
