Knee pain under cap

what could this possibly be?... left knee/cap pain for past month. pain shifts around knee cap vicinity.
occasionally pain shoot down the out side of leg.

no major health issues, no arthritis.47 yrs old; no sports activity. no work out activity.  desk job. physically ok. not obese but 'over weight' by medical standard.

 5'9" (175cm) 91kg. (200 lbs)

left knee cap pain (left-hand side of Left  knee cap)

pain can be described as a 'burn' or 'tender spot' on/around knee cap .  pain feels directly under knee cap and around it.  maybe feels like a tear. no prior knee injuries. no cracking sounds.

2/10 pain with full motion stretching leg after prolonged sitting. 0/10 pain twisting feet around leg axis while sitting. 0/10 pain laying down. 0/10 pain sitting down. 0/10 pain with no movement. 3/10 pain prolonged walking. 4/10 pain with direct touch knee cap area. 7/10 pain kneeling on floor.   very slight swelling compared to right knee-not really noticeable. standing up for prolonged time 3/10 pain; but squatting after prolonged standing may hurt with 7/10 pain.. worst pain 9/10 is when i bump my knee against something hard or kneeling on it -direct pressure on knee cap produces most pain.

Sounds like patellar maltracking syndrome.  Also called "patellofemoral pain syndrome".    Physical therapy can certainly help.  Surgeries do not work for this, so beware of a surgeon who offers a surgical fix.    Perhaps try a topical anti-inflammatory to see if that can get the inflammation down, as you wait for diagnosis and treatment. 
2007 - partial medial meniscectomy
2010 - full thickness chondral defect & adjacent subchondral edema MFC.   Direct result of stupid partial mensicectomy from 2007.
2014 - Subchondroplasty, chondroplasty, unauthorized 2nd partial medial meniscectomy.
2015 - partial failure of subchondroplasty.

the pain sometimes radiates down the outter side of my L. leg

Pretty typical of PFS. Best to see a physiotherapist for a tailored programme to get the muscles back into balance. You say no arthritis, have you had X-rays and MRI to confirm?
