Knee problem, fixable or not?

Hello everyone, I'm not sure where to begin with this so ill just write down some facts about the knee and we'll take it from there.
Short version: my kneecap dislocated completely about 10-11 years ago, since then it has dislocated again around 7-8 times until i finally got sick of it, IT HURTS.
Now here is the problem, try to imagine this.. when the leg is straight the kneecap is out of its "groove" and i have to bend it quite a bit before it "jumps" into the groove.
I can actually feel the kneecap jumping every time i go from straight leg to bent leg, this of course makes it very hard for me to perform any demanding physical activities because you know.. i can literally grab the kneecap and put it out of its place when the knee is only slightly bent.
I tried physical therapy for a while without results, can this be fixed or should i accept that all i can be is a spectator?
