Results from MRI...need surgery or PT?

I am 63 years old and just received these results from an MRI report.  Would someone explain what this report says.  Thanks.

1. High-grade cartilage loss within the medial compartment with mild-subchondral marrow edema at the posterior medial femoral condyle due to a full-thickness fissure.  Associated small joint line osteophytes.

2. High-grade cartilage loss trochlear cartilage with mild subchronral marrow edema.  Moderate focal cartilage thinning patellar cartilage as above.

What is a full-thickness fissure?  Would surgery or PT help with this condition? Thanks.
