ACL injured in a reconstructed knee. Need help understanding MRI
« on: December 04, 2014, 02:40:08 PM »
Can anyone help me understand this? I am not sure if I need to have surgery again or if I can continue playing hockey if I wear a brace and do some rehab. My knee was injured 2 weeks ago in an awkward fall on the ice. The pain was not too bad, and I only had minor instability. I went to the doc and had an MRI to be safe, since I had an allograft ACL reconstruction on that knee in 2005. I am 49 years old and a serious hockey addict. I play 2-3 times per week and do not want to stop playing.

Here is the MRI findings:

There is susceptibility artifact within the tract of the ACL graft consistent with metal artifact. Given the degree of artifact, I cannot confirm a patent ACL, an in fact, I do not see an intact ACl and I suspect it is disrupted, though the artifact limits such evaluation. Posterior crucial ligament appears normal. Quadriceps and patellar tendons are intact. Collateral ligaments and illotibial band appear normal. Bone marrow signal pattern is normal. The lateral meniscus appears normal. The posterior horn of the medial meniscus is diminutive, which could be related to a prior partial resection, partially obscured by susceptibility artifact or metallic hardware near the posterior aspect of the proximal tibia. There is a small knee joint effusion.  No Bakerís cyst. No evidence of occult fracture. When allowing for the artifact, I do not see a full thickness hyaline cartilage defect.

I wouldn't play until you've been cleared by your doctor and physiotherapist as it reads like you've damaged your ACL

Good luck :)
Came off bike onto concrete 9/9/09
LK arthroscopy 8/2/10
2nd scope on 16/12/10
RK arthroscopy on 5/2/15
Lateral meniscus trim, excision of hoffa's fat pad, chondral stabilisation
LK scope 10.1.19 medial menisectomy, trochlea microfracture, general tidy up

KSHAD65,

Get onto Google and learn how to read MRI - Obtain some MRI software -Again Google will have this - Have a good look and come back here to mention what you've found -

I am a patient like you and to an extent I can - I'll leave it there and wait for you to get back ;D -

susceptibility artifact means that the metal in your knee distorts the images making reading them sometimes difficult.  I would wait until you see the doctor to get an idea of when its safe to return to play.
