How tight should I wear my knee strap?

I purchased a knee strap for my knee. How tight should I wear it? Right now I'm wearing it pretty tight and it feels great but I'm gonna run on it tomorrow. Just kind of curious how tight is too tight?

Have you taken advice from a physiotherapist? You could potentially make things worse if it's not the correct piece of equipment. What's wrong with your knee?

My MRI reported I had a full thickness fissure on the medial femoral condyle measuring 4mmx5mm. Is this big? Also am I guaranteed surgery?

And also Mild chondromalacia patella.

I am a basketball ref and I umpire college baseball.

Attached is my MRI results.

I have no pain when I walk, straighten my leg, I can go up and down stairs with no pain. I just haven't tried to run yet.

I'd say 4mm x 5mm is small. I'd be surprised if surgery is needed unless it gets worse.
Personally, I have an defect which is 1.5cm2 or so, it's filled with fibrocartilage, never been operated on, weight bearing lateral condyle. Cartilage defects are quite hard to fix, require open surgeries often with long rehabs.

Have you been cleared to run?

Thanks for taking the time to respond. I haven't been cleared. I've rested, ice, did everythjng including some small exercises with my leg and I feel really really good. I see the OS wed. Did u look at the MRI I attached. Did anything stick out to you?

I have no medical training, just 5 years of knee bother

It says the fissure is unstable, on that basis I wouldn't do anything sport related until you've seen the doctor, you don't  want  to make it worse

Hopefully you get some good news from the specialist today

I know my OS wouldn't even consider surgery for such a defect and full function (he probably would tell me to calm down the exercise and certainly avoid running!)

Went pretty good. He said I have some deterioration of my cartilage on my femur. However it was nothing he was concerned about. A little arthritis going on but for the most part everything looks good.
