PFOA and CRPS - UK surgeon recommendations please?!

I have patella femoral arthritis in both knees, which was grade 3/4 before arthroscopy and debridement last year. Since then both knees have been getting worse. I also developed CRPS in one knee following the arthroscopy. I am on medication for the CRPS.

I would really appreciate any names anyone can give me of a specialist who might be able to look at my knees but be able to take the CRPS into account as well. At the moment my OS will not go near the CRPS, and wants me to wait until all compartments of my knee are badly arthritic, where he will then to a TKR. He won't do a PFJR because of future revisions, and the dangers of cutting repeatedly into the CRPS affected knee.

I'm not keen on surgery myself ::) and am doing as much physio as the knees can take, go to the gym, etc., but the CRPS and OA combination is tricky to say the least. At the moment I have managed to ditch the crutches, but both knees are pretty painful and creaky. I can't walk very far, can't stand or sit for very long, and my left knee is pretty unstable. I'm 41, with two children I'd like to do a bit more with.... I'm self-employed now after having to leave my job, and so far (crosses fingers) haven't had to claim benefits.

I just want some hope that this isn't as good as it gets. I'm in the UK, in the midlands.

Adrian Wilson in Hampshire, Ian McDermott in London, Tim Spalding in Rugby or the team at Stanmore (esp. Tim Briggs) - seem to be strong choices doing the more difficult stuff
