Hemi Cap Question.

Hi, I have been reading about the Hemi Cap implant for defects in the knee. I am looking for more information and maybe some good results from others who may have gotten one or who has researched this. I had a scope in Aug. for arthritis pain and they found I had extensive fracturing and delamination of the articular cartilage on the medial femoral condyle in the medial weightbearing compartment of my knee all other compartments and ligaments were fine. He supposedly fixed me, but I have continued to have lots of pain and locking. I am seeing a new OS on the 11th so I want to be armed with all my options for repair. I have read about microfracture, but have also read horror stories on here from people after microfracture so I am not sure about that as a option. The  hemi cap seems suited for me although I am not a surgeon because it is for people who only have one damaged compartment and that is the case with me, also it is used in younger patients to prolong needing a tkr, I am 33 years old. My biggest question is does medicare pay for this and if so what criteria do you have to meet to be approved? I am in the US obviously so hopefully someone knows about medicare on here. I am a right leg amputee so my left knee is very important to me, I really need this time to be the right treatment for me. Any suggestions out there I am greatful for. Thank You!

Hi Kim

Firstly good luck with your next appointment. I know I've read of someone on here who had a Hemi Cap implant but that was behind the patella I think. Not sure how much sucess he had with it and I don't recall his name. Not very helpful! You could try a search on here. What is the difference between this and a standard PKR of the medial compartment? I'd be asking to see someone who specialises in partials in younger people and seeking their views, evidence of previous successes and so on. I'm not sure how feasible it would be given your situation but has anything ever been mentioned about an osteotomy to unload the medial compartment of your knee and shift the weight to the lateral side if that is in good condition? Might be worth asking about that too. I think with extensive damage as you seem to have, microfracture isn't going to bring you long term relief. Can you push for a scan now to show just how much damage you have and allow them to explore more options? Can't help on the medicare question as I'm UK based, sorry.

Hope you get some answers and good on you for being prepared. Don't be fobbed off, (I know you won't be!)

ROTTENKID is the only person I can remember that has had a Hemi Cap and I don't remember what the outcome was over a year or two.

Hello Kim,
            Just wanted to let you know the little info, that I have about the Hemicap. I had this surgery done 12-21-10, and so far so good.  I am 39 yrs. old and had almost the same damage.  Grade 4 to medial trochlea, and patella was shredded.  I am now in my 3rd. week of therapy and now starting on bike.  Glad to be up and walking, but slow going.  I did all my research on, they have the hemicap implant that they used on me, and my patella was replaced with a Depuy. My only other option was a TTT, which my current surgeon said it is contrindicated with patients who only have damage to patella and trochlea.  My ligaments are all in good shape as well.  I noticed after my first scope that climbing stairs became worse.  2nd scope, I could not do stairs at therapy without therapist cringing at the sound of my knee popping and grinding.  So, I hope the website helps.
