After so long it is possible that the ligaments on the lateral side  have tightened or the medial ones have streached out.  Or both.  It is also possible that you have patello femoral arthritis and the cartilege is not there to help prevent lateral movement of the knee cap.  I have that on the right leg now and can see the kneecap moving toward the outside of my knee when I bend it.  Does this help?

hi
im new to this but like two bad knees not new the the hell of knees, there is also a another possibilty that you may not be as active as you were wich allows the muscles and tendons around the knee to weaken wich in turn allows dislocations but like two said too when ever a knee has been injured or had surgery it is prone later  to the rissk of a cpl of diff arthitus complaints too like two had many many proceedures done and it seems once i first injured them it became a never ending cycle for me. but it may be good for your to see a phsio and they could check the strength of the surrounding structures .
