Arthroplasty failure?

Just had my 3rd scope in 5 years on my right knee. First procedure was in 2005, microfracture and chondroplasty of the femur. And medial meniscus repair. Second surgery was in 2006, microfracture and chondroplasty again. Held off as long as I could and had third procedure on July 2nd. My OS said at post-op visit this was the last time I would be able to have this procedure done, too much damage. He was also worried that he wasn't going to be able to repair it this last time. And the knee is grinding again already. Pain level and swelling was improving but now getting worse again. I don't see OS again until Aug 9th. This time is totally different than the first two. Any thoughts? Did it fail already?
