Patella tendon question

I was just wodnering what it could mean when you have pain on the patella tendon, which comes and goes.  Sometimes it can be not too bad and other tiems I can't believe the pain, then it justg goes away after a fwe minutes.  I had an ultrasound test last year on my right patella tendon that said there was nothing wrong  and now I'm having the first pain there since my scope.  On my left knee it is much worse but never lasts for very klong.  Does this sound like tendonitis?
Feb 08 Inurred right knee
18 months of physio, knee tracking but knee still painful
Aug 09 Scope - Small tear in acl, fragment found in postereo-lateral compartment, suprapatella and lateral plica and small defect in mfc.
May 10 Right knee feeling better but left knee causing trouble as a result o

It could be - I've had patellar tendonitis flareups now and again in the past and the pain levels do vary quite a lot. However for me there was always an identifiable aggravating factor - long downhill stretches on hikes, for example, always set it off for a few days or even a few weeks afterward. Is there anything obvious triggering the pain, or does it happen randomly?

I don't think I've done anything strenuous since my scope to cause tendonitis, so it is a real mystery.  I'm am doing steps, though and some slopes, but nothing really difficult.

Are you taking any NSAIDs? I always found those quite effective for settling it down, as long as it wasn't a really acute bout.

I know you've had problems with both knees - are they still affecting your gait? If you're walking even a little bit off, that could be aggravating the tendon.

If gait is something that can cause it then perhaps it oculd be responsible  I havn't walked naturally since my injury in feb 2008.  I could walk quit wella fter my injury but it was not my normal, relaxed walk.  Since my scope I've been trying to get my old gait back but I can't remember how I used to walk now since its been so long.  I try out lots o different gaits.  My walking is sort of OK but it doesn't come naturally.  Since my left knee flared up I've found it harder to put my left heel down and that seems to be my gait problem at the moment.  If it is my gait that is too blame then it is a very vicious circle, for if I force my leg to walk properly I will hurt it but if I don't I will hurt it so it is a catch 22 situation.  Also when I put my heel down it feels like the leg might bend backwards.  Both legs have dodgy acls.  My left knee has had a dodgy acl since 2004 but is still all there but wasn't a problem until this year and my right acl was injured in 2008 and that has a slight tear in it but is basically sound.  When I first hurt my left knee during asparring amtch when I had my leg swept from under me and fell funny I was rough for a couple of weeks and took a month of Karate, then I was back as good as ever and the injury didn't bother me until the leg was overused since the scope alst year..

P.S. If it is tendonitis would it be swollen and warm becaue mine doesn't seem to be?  Also, it doesn't hurt when I touch it or press it.  It was really bad on friday and earlier int eh week but I havn't had it yesterday or today.  It really comes and goes and seems very random.  Does this still sound like tendonitis and would a pt be able to diagnose it on examination?
