Need some help with my MRI report

If I can I will post the picutre later but for now this is what the report reads.

MRI RIGHT KNEE   

CLINICAL NOTES: Locked right knee. Exclude bucket handle tear medial meniscus.

Some of the images are degraded by movement. ( Trying to keep a locked knee stright down for an mri didn't work well)

There is either a high grade partial or complete tear of the ACL from its femoral attachment. There may be a few tiny intact fibres.

There is also a complete rupture of the medial collateral ligament in its mid portion with heterogeneous material both superficial and deep to the ligament in keeping with blood products.

 The Menisci, exterior mechanism, PCL and lateral collateral ligaments are intact. There is a moderate sized joint effusion. Patellar cartelidge is not well seen due to movement on the axial images.

 There is an acute bone contusion in the lateral tibial plateau posteriorly.

Conclusion: MCL rupture. Complete or high grade partial ACL tear. Bone contusion lateral tibial plateau.

Just wondering mostly about the "Exclude bucket handle tear medial meniscus" and the "bone contusion lateral tibial plateau. Also if anyone has had an injury similar to mine could they share their experiences with recovery.

That simply means "look to see if it is a BHT blocking the knee"

The MCL does seem to be able to heal itself for some odd reason while the LCL cannot. But whether they will offer you a recon or not depends on your specialist

A tear of the ACL that bad even if it is still just a few fibres hanging on, will not heal and they will probably tell you that you need a reconstruction. In view of your age and sports background it is probably the right way to go as currently you do not appear to have any meniscus damage. The amount of forward and backward movement of your tib and femur will change that if things are not addressed.

It looks like it is the ACL that is jamming your knee.

You have a bone bruise which is bl00dy painful and can take ages to go away. It is on a weight-bearing part which means it will cause you a lot of pain. An unloader brace designed to change the forces through your knee may help take some of the pressure off the lateral tibial plateau but whether that is a route your surgeon wants to take is up to them.

They may find more damage when they go in as even "good" MRI's are notoriously difficult to read and they do miss some of the damage. As yours is blurred due to movement I would not be surprised, given the other damage, if there is some meniscus damage. But they have excluded a bucket-handle tear which means they may be able to do a repair rather then a trim which is good news. They will take a good look around when they go in and sort out whatever needs sorting.

Do you know if and when they will operate or will that be discussed at the next appointment?

At least you have a definite diagnosis and can move on to getting things sorted out and back on the road to recovery.

Thank you for your response Kaputt!
 My PT seems to think that my MCL will also need to operated on, at my last appointment we were discussing what the OS will likely have to do and she said "With the way your MCL has healed and the scar tissue has formed around it, he will have to do something with that as well as the ACL recon. He also ahs to figure out what's going on with you meniscus because, there's definatly something going on there."

My question is, will the recon give me a stronger knee then if I just worked hard at strengthening my surrounding muscles ? Why would I have the bone bruse on my lateral compartment if I injured my medial side ?

 My suregon (Dr. Khali Ahmed) is "gone home" for a few weeks and he should be back sometime in the next two weeks. (He went on vacation for 5 weeks and I'm not quite sure how long ago it was) . The last time I spoke with him he told me to continue going to my PT to strengthen my legs, get it straight and walk normal... well I'm getting there lol. he also stated on his "course of action" sheet to: Intensive PT, give me drugs for inflamtion (2 Days left), bracing (dispute with insurance right now) and to re-evaluate when he returns. My pt refered to what I am doing now as "Pre-hab" so I guess she's pretty convinced. Since my kee has gotten slightly better but no signifigant improvement in the first 2 months of my PT, drugs, etc I'm assuming that means that I will need surgery and right now, witht he discomfort I'm in... I'd just assume do it myself :P .
