Can someone PLZ tell me what a focal cartilage defect is????

Hey yall, man I have done went and F**** up my good knee, steped in a hole in the yard, and feel  got cortisone inj for it and it helped some ya know with like the muscle pain and all, but that dang pain on the MFC want stop. Hurts with movement and sometimes at rest.  So OS ordered MRi which showed a large focal cartilage defect (i got the numbers somewher)Theres also a large joint effusion chondromalacia grade 3/4 and a smaller defect under the knee cap.  So whats my prognosis??? Help me out man all i keep gettin is stuff saying OATS, and such....NO......:(

Right Knee:  1983 meniscal tear scope debridment &lateral release. 1991, 1997, 2001
2003 scope meniscal tears. 
8-08 MFX MFC lesion FAILED.
 4-09 Went in for OATS-not a canidate.
 6-09 Uni Right MFC.

From what i understand focal means small. As opposed to global meaning large. What the exact criteria for each classification is i cant say.
12/99 left knee lateral release and plica removal
1/09 right knee chondroplasty and microfracture LFC

From what I've found out re my knee so far, focal means a specific area of cartilage loss rather than diffuse (widespread) loss. E.g I have a focal defect of about 8mm on the medial condyle - it can be pinpointed on an MRI. I also have diffuse cartilage loss behind the patella - as in chondromalacia - its wearing out all over the patella!

So, you've got a focal area of loss which is what they're suggesting OATS for I'd guess and some other loss on the kneecap - which isn't amenable to OATS using your own cartilage but might be ACI suitable....or things might settle down as there is an effusion as well, which could be contributing to your pain....but unfortunately focal femoral defects don't tend to go away on their own.... :'(

Thanx Lottie, yea I know I cant believe I hurt the good knee, man thats some crap  ???.  So my right knee was a foctal defect as well, found my report, dang this sux.  Just started a new job in October but its awesome in that Im not at the hospital any longer .  Its a private physician practice low key most days.  Id say steady.  But man I dont want to have an op already on em ya know.  It hurts bad though man, steps kill me, my right knee no prob but this left one OMG.  Well just venting but thanks for the input :P.......................Later
