Looking for Help, ACL tears
« on: January 24, 2009, 04:24:35 PM »
About 2+ years ago, I ruptured my right knee ACL.  3 months later, I had the reconstruction surgery using a patella graft.  Even with the extensive rehab I still have frontal knee pain and posterior pain as well.  The first OS said to me that "you are still getting better," I left his office that day (6 months ago) and found myself a new OS.
The new OS ordered me an MRI on the spot and 3 days later, I had the MRI.  When I went back in to meet with the new OS, he was very blunt in telling me that the replaced ACL had about 80% of its integrity left, but did not look like it was going to deteriorate any further than that.  He had also told me that the placement of the ACL was "a little suspicious,"  which worried me a little.  He went on to say that it looked like it was too tight and that it was not tracking properly within the knee itself.  Hence the posterior pain and the degrading ligament.
At this point he recommends that I try 2 different braces over the course of a month to see whether or not they help with pain management.  I have tried one of the new braces he gave me and I already notice a big difference.  However, I don't think I want to wear a brace for the rest of my life.  Which brings up another very important point.  Since the brace worked well enough already, OS suggests that a hamstring graft would improve movement in the knee (ROM is good but not fast enough) and there is a 70% chance that the "play" my ACL has would be corrected with the placement of another ACL.
I am curious as to anybody else has had similar issues.  Is there anybody who has had BOTH patella and hamstring grafts?  If so, what kind of pain can I expect with my hamstring?  I really do not want to go through surgery again but I do have significant amounts of pain within my knee and I can't  run around and do the things I used to do.  Any suggestions?
*The brace I am currently using is a Breg Fusion

I had similar problems to those you are describing for the same reasons. Recon using a patella tendon graft - continued pain limited flex and also instability. To cut a long story short I went to a knee specialist and after trying conservative treatment to release the knee, finally bit the bullet and went in for a revision.

Turned out the the first graft was so incorrectly placed that my current surgeon was able to remove the offending graft, fill the holes with my own bone marrow stem cells and still drill new holes for a hamstring graft  That all took place in August 2008 and hopefully on Thursday he is going to clear me for skiing (gently) after I'm fitted with a functional brace. Thursday is the 6 months point.

I tried to work through the problems for 2.5 years and did not succeed. In the past 6 months I have made more progress than ever and can actually see the light at the end of the tunnel.

I also had a lateral collateral ligament reconstruction as well as a postero-lateral corner reconstruction. It might be worth your new guy looking at these structures to see if there is any damage there. I had 4 operations prior to my last one, all of which missed the LCL and PLC damage.

I have not had the brace issue but I have had both hamstring and patellar grafts taken from my same leg.....

Basically with the hamstring graft I found that I was unable to bend my leg backwards at all torwards my bottom and as such you have to rehab your hamstring as well as the knee itself.
Its important to consider they take a long strip of your hamstring ( not just a small section) and after the surgery you will be able to feel and notice where the (large) portion has been taking from.
Its almost 2 years since my hamstring graft and my hamstring which has lost its graft still cannot be lifted as high as my "healthy" hamstring.

Having said that you can function pretty well with both grafts taken and given the option of that OR having a graft taken from my healthy leg, I would have to go with the former every time.
There is not so much pain with the H/String graft just a very frsutrating feeling that it has lost all of its stength... which can persist for some-time.

Hope this helps
