Fluid on the knee

Not me, but my work colleague - a few weeks ago my friend fell up the stairs at work and banged her knee. It is the first time she has shown me her knee which is so filled with fluid and painful it doesn't resemble a knee. She has seen her GP and he seems concerned and will arrange for a hospital appointment, but I'm wondering what she could have damaged to cause such a large fluid build-up. Any ideas?
