Flex and Extension Problems - 2.5 months post op
« on: September 24, 2007, 01:29:06 PM »
Hi all,

Im hoping to get some advice if possible. I had an ACL reconstruction in early July and seemed to be making progress. I now have physio 3 times a week and am 2.5 months post op.

Unfortunately I still dont have full extension or felxion in my knee which is flustrating and concerning. This week my physio is stopping the strenth training and returning me to flex work to try and get it sorted. I can get full extension if I put my leg out and lean into the knee but as soon as I stand or bend the knee it returns to a slight bend, it is never fully extended when In stand, and my physio informs me this should have been resolved by now.

The same is with pulling the leg back towards my lower back. This is painful until I have done it 9 or ten times then it seems to flex all the way back. However as soon as I extend it again for any period of time this becomes just as hard to do again.

I am concerned that I am still stuck in this area as I want to progress with my strength training which is impossible with this movment problem. This seems to be a muscle issue as the knee will move in full range it just needs to be forced into it every time!
Has anyone experienced the same problems, and are there any tried and tested methods of improving my situation? I am concerned I may be doing too little or too much on the knee which is halting my progress. 2.5 months seems quite far along to still be having these issues.

Thanks for your time

Hi Tom,
Sorry no advice as I'm not in a position to give any. My condition was only minor in comparison. All seems to be going fine so far for me.
I just thought I'd reply to your post to let you know that I'm interested in knowing how things turn out for you.

regards Sean

Thanks Sean its appreciated!

I guess I should have mentioned that my injury was slightly more complicated. I disloctaed my knee joint when my ACL went and it relocated as I hit the floor, this shattered a small hole in the side of my tibia which my surgeon decided against fixing in my surgery as he wanted to wait and see if it gave me problems as he said it was unlikely it would.

I am doubtful this is causing me the problems I have now although I guess it could do, I have no medical knowledge however! I guess my issue at the moment is just getting this full range back, its as if the knee is winding me up, allowing me to fully flex it one way then as soon as I move it the other its restrictive again. This must be a muscle problem but Im just not sure how to improve it other than my usual stretching exercises.

Ouch!
I turned my knee when I accidently stood on a fire hose, only caused a small tear in the meniscus but that gave me all sorts of problem. Again I was able to flex my leg in one way but then not another. Just maybe you still got some other damage that is giving you similar problems.

I feel for you.  All I had was a partial menisectomy and now 6 weeks later I am almost getting full extention and flexion.  It still is stiff and I still need a cane in public. Many  people on this site say everyone heals at their own pace.  Good luck.  MY pt says focus on what you can do and have achieved.  Just stay positive

Hi there.

it's possible that the problems you're having gaining range of motion (ROM) are the result of excess scar tissue. Here are some links to info on KNEEguru's main website.

Dr. Millett’s article

http://www.casebook.kneeguru.co.uk/index.php/knee/issues/arthrofibrosis_of_the_knee/

Dr. Noyes’ arthrofibrosis tutorial and Dirk Kokmeyer’s rehab tutorial

http://www.kneeguru.co.uk/KNEEtutor/doku.php

KneeGuru’s literature review (look under problems with healing)

http://www.kneeguru.co.uk/insights/doku.php

I highly recommend that you take some time to read through the info and see if you identify with what is said.

Best wishes,

Jaci

Thanks for your reply Jaci thats really helpful.

I went to see a specialist who works alongside my physio and she feels the issue is the same. She thinks its scar tissue from my op immediately after the injury, that and a lot of sitting around has caused my restriction.

I had my injury in Feb and had an op the week after cause my knee locked due to the bits of bone floating around, ouch!
She thinks because I spent my time before this operation recovering from that one that I simply wasnt moving enough.

Im ready for some hard graft now anyway, my onyl issue is the muscle soreness after I really work on the knee. Anyone know any good creams or techniques to reduce pain and soreness during and after extension exercises??

Thanks all - Tom

I have the same problems as you. I am almost 4  mos post op from ACL reconstruct and cannot maintain extension or flexion. Any gains I seem to make dissapear within 15 minutes. I can get close to full extension when working it for about 10 minutes but lose it shortly afterwards. I have also been told by my OS that I have scar tissue around the knee as well as a cyclops lesion( hence the extension problems). I am scheduled for a scope in November.

Sounds like you may be suffering from Arthrofibrosis as suggested by Jaci. Be sure to read the links she sent you they are very helpfull and you need to be informed and proactive with scar tissue.

Good Luck
