Questions on Medial Patellofemoral Ligam Reconstruction

Hi all

I am two years down the line from having a Roux Goldwaite procedure with a medial reefing and lateral release.  I have been told by OS I now need a Medial Patello Femoral Ligament Reconstruction.

Has anyone had these's done and how was it ?  Very scared of another operation.

Then he wants to start on left knee !   ???

Hi Em,

I've had this operation and was a huge success. It totally fixed the lateral instability of my left knee.

What questions specificially do you have ?

One thing I can tell you that it's a long recovery and requires a lot of physio afterwards. What has your specialist told you ? ;)

Thank you so much for replying kjwilkin, I appreciate it.

My OS has said he is going to do a Reconstruction MPFL from the medial epicondyle to the medial patella.  He will make 2 incisions, take the hamstring ligament then pass it through a patellar tunnel.   He said he cant drill into the patella rim to fix a screw as my patalla is too worn on the inside.  So he will attach it back to itself and suture it.
He said I will be in hospital for a couple of days.  Then brace for four weeks with partial weightbear.  Then after 4 wks he will wean me off brace.  He said Physio is depending on progress as to when start.

I am scared of surgery.  I dont do well with Anaethetics, vomiting etc.  Plus I remember the pain of the Roux Goldwaite procedure.

I suppose my questions are, will the pain be as bad as the previous surgery ?  Will the time on crutches be the same ?  My quads and vmo are still weak from last surgery, will that cause problems for this one ? Can my left knee cope with my right in a cast again as left has started locking and crunching ?  ( I am awaiting an appt for left to be examined)

I had this surgery a year ago.  It is a very hard operation and I did have a lot of pain, but that lasted for only about 2 weeks.  I was parial weight bearing from the beginning, but because I had trouble with regaining muscle control because my leg was very weak I was on crutches and in the post op ROM brace for 6 weeks then in a smaller patella brace for a few more weeks. 

Good luck.  There are a couple of active threads with people that have had the MPFLR so you might want to look at them to see more of their experiences because the post op protocol seems to vary a lot for this operation.

Dec 00 - L knee TTT
May 02 - R knee TTT and VMO advancement
Dec 04 - R knee screw removal
still too many subluxations and dislocations
July 06 - R knee MPFL Reconstruction

I have heard its a hard operation and after my last one that scares me.

I have read a couple of the other treads and even though they have had problems, the general concesses is its a good operation and it works.

I suppose I just dont know whether my other knee can cope doing all the work while I am on crutches. Also whether I can cope with having another operation.

Hi Em,

Sorry it has taken me a while to respond. Still trying to come to grips with what my knee surgeon said on Wednesday.

I had this operation and was a huge success. Before the operation I couldn't walk or do much so it's pretty amazing what I can do now.

The operation is painful but the pain usually settles down after a few days. As for crutches, it is a very individual thing. I remember having to wear the brace for a couple of months. My quads were totally wasted before the surgery but have worked really hard to get them strong now. Any knee surgery is going to make is weaker. My knee surgeon says that my muscles are quite strong now, which is great. I'm sure your other knee would cope with it. I started physio pretty much straight away after the surgery which was really important. It is a long hard road, but definitely worth it in the end. My knee surgeon said it could take up to 3 years to come good and never really believed him, but it did.

Presently I've got medial instability of my left patella and my knee surgeon said he doesn't think he can do much more. Was absolutely shattered but coming to grips with it slowly now. My sports physician, knee physio and sports psychologist are going to meet to discuss some other treatments. My knee surgeon said that if my knee radically changes or if he sees something that he thinks will work he'll let me know. At least he hasn't closed the door completely. He did mention a new brace to try that has only just been released in the last couple of months. So, I'm going to give that a go.
