Patella surgery in three weeks, wondering...

Hi.
This is going to be my second surgery on my left knee. In 1996 I had arthro for a menisectomy. Developed chondromalacia, fairly significant, over the years. Sports, wars, etc. Now, slammed my left knee two months ago, directly on the patella. Pain gradually increased. Could not run at all after four weeks. Went in last week. MRI revealed 'loose body', possibly piece of my patella separated, being held on by muscle, tendon? (I have always had 'tri-partite patella anyway..doctor says my anatomy is weird to begin with) Doctor unure exactly, but knows he needs to get me in. (he's worried its just going to 'give out' on me) Said something about me not being able to do a straight leg lift as the body is telling the muscles 'don't do that, as it will cause more damage'. Maybe falling, the tendon pulled the patella piece right off the rest of it (?) Talked of either going in with the scope first, if it was repairable perhaps trying to pin the fragment back to the rest of the patella. Or, if not, removing the piece altogether. I've had three surgeries altogether on the knees, 2 L, 1 R, and had a C5/C6 diskectomy and fusion with instrumentation last fall - so major surgery does not bother me. But, this guy is good, and he did not say much. I asked if I could work/drive after 4 - 6 weeks. He said 'MAYBE six weeks'...I am wondering what the recovery, recuperation is on something like this. He mentioned something like a knee immobilizer, that will make it difficult geting in and out of a car even...I get my surgery packet soon anyway, but since I enjoy reading about this kind of stuff anyway - I thought I'd throw it out there.

hi jc,
as your not sure what the surgeon will be doing exactly i'll tell you what ive had done as it may be similar............i had an accident where i landed directly on my patella in a bike accident,ive just had my second surgery on it,im now 4 and a half weeks post op i had similar op to you the first time but as this could not correct the problem was advised i needed "bigger" surgery which is the op i have just had,my surgeon gave me a "brief" description of what he was going to do but advised until he "got in there" would know more as to what needed doing.(my patella had moved outwards to the outer side of my leg/tilted upwards at the top and inwards at the bottom)

i had what he called an "open tissue patella realignment and lateral release"
i have x 2 keyhole incisions and an approx 3 1/2" incision.
i had still got my brace on and have had to have it on since the op and for 6 weeks in total.
my brace is an irom donjoy which if you look on the net is pretty horrendous looking!! (DonJoy IROM Knee Brace -- JointHealing.com)
the first fortnight it was set so i could not bend my knee at all,this was awful as i had to basically stay off it as close to 24/7 as i could so the knee cap could stay in place,after this it was set to 45 degrees.

i feel i was prepared for the fact i would have a brace on for the six weeks but unfortunatley do not feel i was prepared for how hard it is having it on,and the lack of movement i would have.as i don't know which you will have on i can only advise on mine!!
i was hoping after the 6 weeks which i only have 1 1/2 weeks to go that i would be able to do more then i feel i will be able to,my ROM is only about 30 degrees if that and i can just weight bear but feel better on my crutches as im very wary about doing too much on it as i twisted it the other day and it swelled alot!
i would not like to hazard a guess when i will drive again after this op as i can not bend my leg enough yet to get in a front seat of a car as a passenger let alone do an emergency stop (its my right leg) and wish i had been told this before the op so i could have prepared myself better,it did turn out to be a bigger op then he first thought as it was meant to be day surgery but i had to stay in as i was in alot of pain when i came round.

i will have to have physio shortly to get my ROM back which i feel pretty daunted by!
i was initially given six weeks off and have to see my gp this week to extend my sick note as i do not see my specialist/surgeon til 6th march,i have been advised i will be off for approx 12 weeks,but was not told this initially!!
if the op turns out to be with the scope/keyhole then this is completley different i had this the first time,i was put on sick for 2 weeks and felt fine and was driving 8 days later i had no brace for this though.
good luck with it and let us know how you get on!! i would speak to him again if poss before the op and ask if he can be anymore specific and at least then you know what you are preparing yourself for!!

Yes, the doctor kind of talked about not knowing until he 'got in there'. The first course, he thought, would be pinning the patella piece back to the remaining patella. If not feasible, he said the 'easy part' would be just removing the patella piece, and that would be it. But, even after inspecting the MRI images, he said it was 'up in the air' as to the exact goings on in the knee, and he had to get in to look. I have six weeks or so sick leave on the books, so the time off is paid, but I am just going to go stir - crazy at home! The doc said something about a knee immobilizer for four to six weeks...A big part of my job is driving for casework and interviews/investigations, so driving ability is a must when I go back to the job full - time. Right now, if I can at least drive (my left is the one getting done), then I can still do my job on crutches or with a cane...Thanks for the reply!  :)

HI THATS OK!! I KNOW HOW YOU FEEL ABOUT GOING STIR CRAZY STUCK IN AFTER THE OP, I AM TOO!!
I WAS OK TO START WITH BUT THIS WEEK HAVE BEEN FINDING IT REALLY DIFFICULT AND MISS DRIVING SO MUCH!!
I AM HOPING THAT ONCE IVE GOT THE BRACE OFF I MIGHT BE ABLE TO GET MY ROM BACK!
