Hi all,

I apparently have Patellofemoral maltracking and have been receiving PT for the last 10 weeks with no noticeable improvement, maybe even possibly got worse.

I have a question that I hope you could help me with that I think may be related to my maltracking but have a worrying feeling it might be something else.

I have a strange feeling in my knees, it does not hurt but worries me as it does not feel right, it is hard to describe but I will give it a try.

when I put my hand on my knees when in a sitting position and move my feet up and down I can feel a strange feeling in my kneecaps, possibly on the top of my kneecap, it is a hard feeling to describe but feels like something vibrating, grinding or slipping, it also happens with my feet flat on the floor if I am to move my legs slightly. I notice it most when I am driving and I put my hand on my knee and press the accelerator. I asked my physiotherapist about it and she said that it was cartilage on the underside of my kneecap grinding on the grove it tracks in, but to be honest I don't agree with her as it does not feel like bone grinding, I have been looking at pictures of the Knee joint to try and figure out what is causing this feeling and I think that it is my Patella Tendon or the Tendon bellow the patella slipping on my patella.

Anybody got any ideas what this could be and if it is related to my Maltracking, it is a horrible feeling and seems to be getting worse so I have stopped my exercises until I can see my physiotherapist which is late next week.

Any help is greatly appreciated.

Right

Is there any clicking of your kee caps when you bend and straighten your leg?

Also as part of your Physio have you been stretching your hamstrings and quad muscles?

Thanks for you reply Chris.

When I first started to have problems with my knees they would click or crack every time I bent down low, but now I never bend down low because the lower I bend the more pain I have in my knees, especially if I squat. Sometimes when walking my right knee clicks with every step I take which does not feel nice, I also get a lot of pain in my knees if I sit for long periods of time and usually have to sleep it off for them to get better.

I noticed the other day that when I now bend my knees slightly while standing my left knee makes a crunching or grinding sound which cant be good.

As for the Physio, they have been giving me exercises to strengthen my VMO muscles and that is about it. One of them which causes me pain and discomfort is, I stand with my back to the wall with my feet about 1ft from the wall and have my feet shoulder width apart and hold a towel between my knees, then slide down the wall as low as I can go before feeling pain and hold for 3 second then repeat 10 times 3 time a day. I especially get that strange feeling if I touch my knees while doing this exercise.

Hi Jay

I was told for months i purely had a tracking problem and VMO exercises were the way forward but i was not satisfied and saw different people it turns out i had a complete muscle imbalance in my legs and back.

If you can my advice would be to see a Podatrist and request a biomechanical assesment they will access which muscles are too tight etc.

Then you will know which ones you need to strengthen and which you need to stretch.

Also this website may help you understand how your body works biomechanicaly.

This is going to sound very harsh on the medical profession but in my experience dont take their word for it even the Specialists if what they say does not correspond with your own common sense ask more questions and if you dont get the answers see someone else.

Are you stretching your Quad muscles and hamstrings?

You only get one body.

Hi Chris,

What you have told me sounds very interesting and gives me another angle to investigate.

When I first met with the doctor at the Physio center I use he did comment that I had flat feet and this could cause problems for me in the future. I went to see a Podiatrist just over a week ago to have my feet looked at, after assessing my feet and the way they move he did see a problem and made me up insoles for my shoes but said that I should consider having insoles molded to the shape of my feet as I have strange shaped feet. I will contact him soon and arrange a biomechanical assessment as you have suggested.

I have to admit this problem is starting to get me really down, been off work for about 3 months now as my work is quite physical and involes a lot of bending down, also doing pretty much no activity at the moment as this aggravates my knees and causes me pain :(

Its the uncertancy of the problem that worries me the most, at least if I was 100% sure of the problem I could get on with what I have to do to rectify it, if it can be rectified.

You sure are right about only getting one body, its amazing what things in life you take for granted, simple things like being able to walk freely with no pain or discomfort, times like this make me relize that your health is one of the most important things in life.

I have not been stretching my quads or my hamstrings, do you think this may help? Also have you had the same feeling in your knees that I described in my first post ?

Jay....I have been dealing with a maltracking problem now for 16 mos. I have very much the same symptoms and feelings you have. It is so hard to explain, but it is most definitely a weird feeling. Long car rides are the worst. When we stop, and I try and get out of the car, the pain and feeling is just tremendous. I keep saying it feels like some thing is moving around in my knee. Recently, when I walk, it actually feels like it moves side to side. I have been through rounds and rounds of PT, and this is what they feel it is too...maltracking....and say if its not fixed soon, it will cause more problems to my knee. I see pain management next Fri. and have found a surgeon who says he can do a reconstruction. Your story is soooo similar to mine. Keep me informed. I need to run for now, but will check back later on....

Jay, I have the same problem as you, but only in my right knee.  I was diagnosed with bad maltracking a few weeks ago but i have been having the same feelings and pain in my knee for the past 6 months. My knee also feels like it's sliding out of place.  How far do you have to bend your knee before it feels like it's slipping?  For me all i have to do is just barely bend it when walking and i can feel it crack and slide/pop out of place but it goes back when i stop walking.  When was the last time you saw an os/ have you ever seen one?? you may want to think about having an os look at your knee for a true diagnoses. How long has your knee been doing this??  My os told me that since it's been 6 months of conservative (i've been doing pt since december) that it's time for me to get an arthroscopy done along with a lateral release for my maltracking (I'm going in on monday for the surgery).  you might also want to start strengthening your quads, it has helped me some but it's just not enough, so maybe that'll help you. Hope you get your problem resolved soon!!

Just have to add that my PT's have been able to notice the maltracking way more than the OS. I think this is because they work with me on a one on one basis several times a week, and with doing various activities...so they know better.

Hey Donna and Ashley,

At last after looking at just about every thread on these boards and surfing the Internet for hours I have finally found some more people with the same weird feeling in their knees as me, all we need to do now is find some answers as to why our knees are doing this.

I see my Physio at the end of next week, so will ask more questions and let you know what they say, I am also going to ask to see the doctor again at the Phsyio center I use as my problem is getting worse I think, I would also like to ask him some more questions.

Ashley, I only need to bend slightly with my body weight on my knees to feel my left knee grind or slip, I am lucky and cant feel it when I am walking but if I walk more than my knees can take in a day I start to feel my right knee clicking with every step I take, my knee also seems to get really tight when this happens and is quite painful if I try to bend my knees properly when walking.

I have not been to see a OS yet as I live in Britain and have the misfortune of having to use the NHS, so it looks like I will be waiting at least 6 months before I get a appointment with a OS unless I choose to go private which I cant really afford but what price can you put on your health?

Good luck with your surgery on Monday, I really hope it all gos smoothly for you, please keep me informed with how you get on  :)

Jay...Just curious about something. Does your knee hurt alot when you walk up even the slightest elevation? Such as a ramp? Mine does...and slips away. Even just sitting here at the computer, I can feel it move in and out of the groove. It is so frustrating trying to get these doctors to just correct it and quit playing around.

Hey Donna,

I have not been walking up any elevations so cant tell if that is happening to me, if you say you have had the problem for 16 months then maybe your condition is worse than mine as I have only had my problem for about 4 months now, I will try and walk up a elevation to test it out  :)

I spent a lot of time last night on the internet trying to find more answers and came across a condition called PLICA SYNDROME which I found very interesting as I it seems that a lot of PLICA SYNDROME cases are wrongly diagnosed as Patellofemoral conditions.

I used to get that painful slipping, skidding feeling while driving. Sometimes I wouldn't notice the slipping so much as the click as the patella bumped back into place. I dont' have that since my TTT.  With a very slight bend I still sometimes have the skidding but not like I once had.

Hi Jay,

Have you had x-rays to confirm the maltracking?  If not, it might be an idea to get some done as this "usually" confirms a maltracking patella.  I wonder by your user name if you come from Edinburgh? (I'm from just outside Edinburgh).  If so, there are some really good knee specialists at the ERI, my surgeon, Mr Macdonald, works out of there and I would recommend him, he does NHS work and private and I've seen him through both avenues, obviously private is quicker, I can name a couple of others if indeed you are from that area.  I had the same kind of symptoms you're having with the slipping, crunching etc, I do have severe maltracking along with loads of other problems, sometimes when you have maltracking it doesn't feel like bone on bone when the cartilage is grinding against the bone so I wouldn't rule out maltracking as the cause, if you haven't had any x-rays yet I would recommend getting some done and then take it from there.  By the way, PT also made my symptoms worse, it wasn't until I had the maltracking fixed that physio started to benefit me.  Good luck, hope you get some answers soon.

PS:  I would try to stay away from doing the wall squats, my pt told me NOT to do them until the maltracking was fixed.
