Who has had a Patella Femoral replacement?

I am 38 and strongly considering a Patella Femoral replacement.  I have a few questions for those that have already had it done.  How would you rate the success of your replacement?  Can you perform any type of sports like biking or skiing?  Are you able to go up and down stairs now without holding onto the rail for dear life?   Did you have a custom made (sized to your joint) implant or a standard off the shelf implant?

Hi Rob,

I saw my consultant yesterday and have been advised after a failed ACI back in August of last year to also have a Patella Femoral joint replacement. I was lucky to have spoken to a lady who had the surgery a year ago and she does snow boarding and water boarding and is pain free. She has said she gets 120 degrees of flexion and has had no problems.

She has said that she also manges stairs with no problems what so ever.....

I am seeing a doctor on March 9th for consideration of this type of surgery.  Most of the doctors I have talked to usually perform medial or lateral replacement and none have actually performed a successful Patella Femoral replacement on someone around my age.  I'm 27.  The last guy I talked to said he did one on someone close to my age, but had to have a Total knee replacement a year later.  It makes me skeptical.  However, I am heading to a doctor that may have other possibilities for my knee.  Good luck

I have had a patella femoral replacement

I am 33 with a history of patells maltracking and dislocation which distroyed my trochlear groove and patella. 
I had my first PFR may of 05 and the second Aug of 05.  It has solved a lot of problems, although not the chronic dislocations.  If I did't have dislocations, I would recommend it.  With the continued dislocations, the plastic patella is now a problem.

Hi,
I had a pfr march 28, 2005 so it is coming up on a year now. Now, I am older -- 62 -- so my case may be very different from yours. My knee is still sore but I don't have the sharp pain in the knee that I had. Still, I get pain up my thigh and pressure and pain in the lumbar, lower back region. My other patella is about 70% gone from arthritis so my gait may be way off and causing this.

There is a wonderful young woman on this board called Leta-Montana. She had the pfjr almost two years ago, I  believe. She is doing really well and even going dancing! She is very knowledgeable and helped me a lot when I was considering this. Search for her and pfjr on this board and you will probably find her.

Very interesting. 
I have been blaming my back on my bed, but my lumbar region has been killing me.  Could it be related?  My husband feels it is just due to my "unusual" way of walking.  Very interesting. Maybe I will ask Knee guru about this.

I'll be eager to hear what the knee guru says. I never had lower back pain before the pfjr.

As for beds, we have a wonderful one--a king Tempurpedic. It is so comfortable and supportive, it makes us not want to get out of it! So I know my problem is not the bed.

Let's hope we can find an answer to this back pain.

I really appreciate the feedback.  I have been on a donor list waiting for tissue to have a Bipolar Osteochondral Allograft but it does not look like I will find a donor due to my large joint size.  I am about ready to try the PFR.
From what I have read it is about a 50% shot at success.

I am just starting to investigate having a PFR. My OS has just referred me to a replacement specialist because he thinks this is my next step. I see this new doctor on May 23.

Hi,
I am a 35 yr old male with bilateral PFR.  I had both knees operated on at the same time (Oct. 31, 2005).  I am am coming up on 6 months post op.  Let me tell you it was the best thing I have done for my knees.  (I have had each knee operated on 3 times in 22 years).  I can ride a bike (no sking).  I am able to walk up stairs without holding on to anything.  Going down stairs is another story (I have to hold on to something or use a cane). 
The first 6 weeks after surgery was not the greatest.  I had a fantastic OS and a great Physical Therapist.  My PT pushed my every time I went in for therapy (just alittle more, and a little further each time).  The discomfort and minor pain was well worth it.  Now I am able to walk without a walker, crutches, or a cane.  (I have a young son who I am able to play with and carry around).
All of my problems that I had prior to my operations are gone (poping, dislocating, swelling, extreme pain and discomfort).  I am doing more now than prior to my operations.  I am still going to physical theraphy twice a week.  My quad muscles still need to be worked on (build up strength).  I will need to do physical therapy on my own soon.  I was told that I need to wait a year to see the real results of my operations.  (Every day you achieve alitte, but if you add up all of these baby steps, at the end of a month you can see great results).

Congrats on your wonderful results! At the rate you're going, you'll be gettin down on the dance floor by the time you hit your one year date.

I'm considerably older than you and I only had one knee done. The other one waits till my first PFJR is at peak healing. I am a year out. It's likely that the soreness and sometimes pain I still have is related to my gait stemming from the other knee which is 70% gone. I certainly feel new stability in my op knee.

I am turning 40 this year and have had realignment surgery with lateral release in Nov.  it did not help me at all, in fact I am worse than pre=op.  The next step is a patella femoral replacement.  I am skeptical of this, as it only gives me about 20% more success rate than this past surgery and then on to a total knee.  I am doing as much research as I can before doing it.  let me know if you find out anything interesting.

I  would be happy to answer any questions on Patella Femoral Replacement surgery.  I am 34 years old, This week my right knee is  1 year post-op.  August will be 1year for the left.  It has been a difficult year, but I am improving.  I still have a lot of pain from the surgery itself, but I also have had many surgery both before and after.  I no longer have the terrible grinding that was so painful and limited my driving and made stairs so difficult.  PFR did not help with my chronic dislocation though.  The arthritis pain is greatly improved, but I do have more arthritis which will lead to a TKR within 10 years or so.

You say that your arthritis pain has greatly improved, but you have more arthritis which will lead to a TKR. Where is this other arthritis? Is it in the other compartments in your knee? I'm asking because my medial and lateral compartments are pristine, no damage at all. My only problem is behind the patella (not even the groove). This began with a knee injury seven years ago, and the position of my patella is baja (low toward the shin) because of excessive scar tissue that permanently changed the length of the patellar tendon.The low position causes the patella to rub in the wrong place. I have a lot of questions for the replacement specialist on the 23rd, mostly about the patella baja and my history of arthrofibrosis. But I thought maybe you could explain why you will need a TKR. And why you still have pain from the surgery one year post-op?

I am also very interested in the same questions that Janet has. I am supposed to have another pfjr on my left knee but my fight knee over 1 year post op is still a problem. I have the feeling that TKR is in my future but I am trying to get a fix on all of this.
