Possible TKR

I visited my os yesterday and was told I need a TKR, os told me he would do surgery but I was still really young for this procedure (43).  I opted for the more conservative, try Cortisone Injection, if still in pain will return in 2 weeks and will try arthroscopy to see if he can clean out any loose bodies and relieve pain that way.  He said my joint space is pretty good but I'm still maltracking and have severe chondromalacia.  Already had a maque on this knee and a hausher and maque on the other knee.  I'm in a world of hurt today, probably from the cortisone, and am now wondering, is this all a waste of time and should I just take the big leap?
