medial knee pain

50 yo female, w/ hx of r tibial plateau fx 1998, since has become distance road bicycler, with subsequent piriformis syndrome, ITB and now medial knee pain just about where the mcl would insert on medial femur.  All one problem? or three? medial knee pain the worst of the three, any ideas on dx?
