I am trying to find out anything I can on this Supramalleolar medial rotation osteotomy.  My tibia is at an external angle, basically I'm pigeon toed.  The problem is that because of the rotation my knee does not track well at all. I have already had the Fulkerson procedure done 2 years ago with little success.  I have no idea if this surgery has another name so I can research or if anyone has had this procedure.  I know that the prospect of having my entire tibia rotated and plates is scary, especially with all the pain and trouble I had with the Fulkerson.  Anyone that knows anything would sure be helpful.

Hi

I had a TIB-FIB osteotomy done 2 1/2 years ago. It was done to correct an externally rotated tibia that was rotated 40 degrees from a poorly set TIB-FIB fracture. At the time I had a intramedullary rod down the tibia and a plate on the fibula above the ankle. Those were removed and both bones were cut in half, the leg was rotated back in correct position and then the tibia was plated with 2 plates and screws and the fibula was left alone to grow a bone bridge between the two cuts. Some doctors will wire this. I was in a cast and non weight bearing for 3 1/2 months and a custom made fracture brace for 1 month after that. Pain wise , it wasn't anymore painful than the De Lee osteotomy I had done on my knee. The most painful was having the rod and plate, out of the 7 surgeries I've had. I had alot of problems with my knee from the external rotation of my tibia  that caused permanent damage to my knee.

I appreciate the reply.  I had slammed my knee hard into a concrete floor and the problems keep mounting.  The lack of mobility is what gets me the most.  I think the thing I was most worried about was getting around (cast Vs Brace makes a difference)  Do you know of and web sites I can look at?  The more info I have the better I think I'll feel.

I don't know of any web sites, you can try wheeless textbook of orthopaedics, Entrez pub-med or medscape.
I had to rely on the info my doctor gave me. I went to a specialist who only took care of the leg below the knee. I also saw a trauma specialist who referred me to this specialist. Malleolar refers to the ankle, medial to the inside and osteotomy is to cut bone. So I imagine the cut will be just above the ankle and your leg rotated towards the middle. Which is how mine was done.
