NOTES - Surgical Procedures :
mpfl recon - - Posted by rebeks (rebeks), 1 December 2004
Hi. This is my 3rd surgery reccommendation (TTT & proximal realignment being the others) and it makes the most sense to me. My L patella dislocates and has been doing so for about 10 years. I first remember it subluxing at about age 8 and am now 27. I am a professional ballet dancer and have recently (this past summer) had to stop dancing because of this darn knee problem. My knee is fine now except when I do dance, then I have pain, but rarely instability. But I am afraid to "really" dance. Is it possible for everything to heal itself and I could do more pt now and be ok to dance? Or is surgery better? I am so afraid of being laid up and messed up for so long after and of being worse off than now after this surgery. Please write and tell me your experiences with this recon! And- do you think TTT is usually needed too? I have decided that surgery does not make much sense to me. Please respond with any info, especially with how to make sure it is the right surgeon. Thank you!

Which surgery depends on your specific anatomy and pathology. The predisposing factors include trochlear dysplasia(shallow femoral groove) increased Q angle, deficient medial support (medial retinaculum/MPFL), femoral torsion and so on. Basically you need a thorough evaluation be an expert in the field who has the whole quiver of procedures in his/her armamentarium and the exprience and sense to know when to use each.
These people exist. I suggest Googling International Patellofemoral Study Group for some names.

Dr. Bauman in NYC is supposed to be great. A friend of mine is a professtional dancer and he did her surgery and she can dance just fine now. He specialized in doing knee surgeries for dancers so he is one you should really look into.
Yanks
