Question :  meniscus contusion??

Anyone had a meniscal contusion? It is on my CT scan protocol with a big ? but my OS says he's sure it's a contusion. Somehow my meniscus was "trapped" when I hit my knees onto the dashboard. Also strectched my PCL. There's quite some fluid on the posteromedial side of my meniscus but becasue of the fluid they couldn't be sure if there was a tear. I don't think so.

I have pain when my leg is in full extension. Sitting with my legs bend @ 90 degrees is painfull, esp when I point my foot in or outwards. When extending my leg after it being bend it's extremely painfull, after 5 minutes or so most pain is gone. Waking up in the morning after I have been lying with my knees bend all night I need 10 minutes to extend my leg... Pain is located IN the knee and in the back of the knee, like from the medial side over the back to the lateral side. Should get better by PT, am doing exercises @ home, didn't play badminton as I don't want it to get worse or so.

Anyone had this?? I know the medical story (am a medic ;) ) but like to know if anyone had something like this??

I think I've had similar pain. If you imagine a cross section of the knee as a clock, with the kneecap at noon, my pain starts around 12 and backtracks around ccw to around 4 pm, related to the right knee, medial/posterior pain.

This was the orginal radius of the pain I had when it was determined that I had detached my medial meniscus' posterior anchor - yup, tore the meniscus loose from the tibia in the back, with the meniscus intact, along with a stretched ACL.

The pain was not fun! It hurt all the time, and the more I walked the worse it got. I had a repair using suture anchors, and that was successful. Unfortunately, I had a slip and fall on crutches 16 days postop, and tore the meniscus right through the anchors. So, it was back into surgery for a partial meniscetomy, removing the posterior third, leaving a rim on the posterior edge, anterior section intact, and most of the middle section intact. Subsequent retear a year later, ended up with another partial meniscectomy 18mos after the first one, as I tore the anterior/middle part that was left almost in half. End result: 25% meniscus remaining anteriorly and a rim in the back (which does zip), leaving me with DJD already, and a medially collapsed joint, along with lateral tracking and chondromalacia, plus chronic pain and recurrent swelling.

So, you can see that my knee's a wreck. I think my pain originally is like what you describe. It might be worth trying one cortisone/anesthetic injection and see what happens. If you get some relief for a while, and the pain comes back, then you might need a scope to go take a look. My first OS blew me off about the detached meniscus, didn't see anything wrong based on the MRI, even though the MRI said possible ACL tear. My 2nd (current) OS said live with it or he could go check it out, and the scope showed stretched ACL, which got successfully shrunk with the "wand", and the detached meniscus, which I was informed was an unusual injury. Apparently it's pretty rare for it to tear loose from the bone without tearing the meniscal body.

I hope my experience helps you some. I would definitely pursue it. If you have patellofemoral pain from the "dashboard knee" syndrome, a Tens might help with that portion of your pain. I use one on a regular basis to zap myself and desensitize those nerves, thus reducing the pain under the kneecap w/o pills. I fell on my knee cap about 2 weeks ago. Swelling on both sides of my knee began to increase and the pain was unbearable. It reminded me of a bad tooth ache.
It hurts any time  walk or bend my knee. It makes a horrible clicking and tearring sound when ever I move it.

So I went to my Family doc. He had an MRI done which suggested that  I might have a meniscus contusion or cyst.  Now I was refurred to a OS.  Can anyone tell me if they  were to have Surgery for this, which is being suggested what is the hospital stay and what other things should I excpect. I am a newbe at this sort of thing...

P.s  about 8 years ago I slammed that same knee in a car door and screwwed it up but never did anything about it!  I was in so much pain but just pushed it away and now this fall and that pain are coming back to haunt me!

Whoo DM this is interesting! What did your scan show?

Pain seems very similiar indeed. It has been more than 5 weeks now and it hasn't got any better. Tried badminton yesterday but I could not run or jump :-[. My fellow players saw something was wrong and thought it was my TTT knee. But nope, so that made 'em think something must be wrong as i have always been playin gno matter what and now I couldn't move @ all.

Was extension also painfull in you? I have a very hard time extending the leg and walking is painfull. My OS says to wait and see untill i am done with these 9 PT sessions and then maybe redo the scan. Pain hasn't got any better so far. Am sitting now with leg bend 90° and I have pain, I cannot turn my foot or I could scream. I can walk but am limping. Dunno what I can do. I'll wait and see, ask a fello OS for his opinion when he comes up today. (if I am alone with him and if he has the time, i don't want to bother them or look desperate :-/).

What exact symptoms did you have? plz let me know!

I'll PM you later as I have to go to work now (and if I survive :P)

Just mailed my CT-scan report/protocol to an online knee specialist and he says that according to my symptoms and my report I most likely have a torn meniscus and I should ask my OS for a scope.

Dunno what to do next, wait and see for 1 but if it doesn't get better I think a scope is in order...

Another Q, do you guys think it is wise that IF I have a scope, I ask my OS to do a LR while he's in there, along with the mensiscus-repair or removal?? I said IF, as I don't know if and when I'll be having a scope...

I don't know about that. An LR isn't something to be taken lightly.

You could try a cortisone shot and see what happens. I got a couple months relief from one, then had one more, and got a couple more months, then ended up getting it scoped. Cortisone's not a fix, but could give you some time with less pain, and if the pain returns, it's another clue that they need to go look in there.

I really don't want cortisone in my R knee as I had way too many injections in my L one already and it only made the arthritis worse. I know it's a temporary fix but cortisone has many side effects... I'd rather have a NSAID-injection IA (still have a flacon with Bufexine in my locker @ work :-/)
It's hard with 1 bad knee and a not-very-good-knee, now have 1 bad knee who is even better than my good knee... What worries me is my R knee wakes me up @ night, I cannot straighten it out without pain...

Thanks Donna, I dunno IF he'll do a LR along with a menisc scope, I don't even know if I'll need a scope for my meniscus, but if I do, I'd rather have both things over and done with ;)!!

Ok,

This doesn't seem to get any better, I am debating on seeing my OS again, but I won't untill next week (well, officially then, I see him everyday :P). I have an exam next week and am not having any time to study whatsoever, let alone I would have to go in for more examinations (which i don't think he'll ask anyway :P).

It's just not getting any better. It doesn't feel right at all, and I mean I never had this type of pain before! Not even on my L (**** knee which is now doing really good! :D). Bending hurts, extending hurts, when my foot isn't straight it hurts... Then again, walking stairs is  no problem! Weird huh?

Anyone had something this strange? Somehow I don't belive it has anything to do with the accident. But I don't know how else I could possibly have injured it? I am debating in calling my PT tomorrow so she can have a closer look... I am taking painmeds and this is not my usual me...

Hope I found out very soon what this is!  :'(

It would be so wierd if you end up finding out you have the same kind of injury I had, considering that my os told me its pretty rare.

I hope they can diagnose you or just go do an exploratory scope, and make it better.

For anyone who's interested: (just printed them out today, R knee is the one with the big problems just now...)


L KNEE

Ex. without IV or IA contrast

Status post patellar surgery modified Maquet/Fulkerson (infiltration of Hoffa's fat pad)
Presence of some fluid on the posterior side of the medial and lateral meniscus and on the popliteus tendon.
Presence of fluid on the posterior side of the posterior cruciate ligament.
ACL and PCL are intact (cfr. Saggital reconstruction images)
No associated bone damage
Lateral patellar maltracking and lateral tilt (see history, to compare)

CONCL

Presence of fluid without an obvious tear...
Lateral patellar maltracking/tilt/PF dysplasia (see history, to compare)


R KNEE

Presence of fluid on the medial meniscus, no obvious sign of a tear
Normal lateral meniscus
Fluid on the popliteal tendon.
Normal ACL/PCL, normal lenght, continuous, no significant fluid periligamentarius
MCL and LCL are normal
Normal patellar tendon
Lateral patellar maltracking, lateral patellar tilt/PF dysplasia (see history, to compare)

CONCL

Abnormal findings on the medial meniscus. Meniscal contusion???
Lateral patellar maltracking (to compare)

Again, no improvement today, almost no sleep last night, can't sit down comfortably, R leg continuously hurting "inside".
Never mind the patellar maltracking, had it for ages and will always have it :-/. It's not causing my present problems (I think)...
