Multiple Recent patella dislocations

Hey guys I dislocated my patella a little over 2 weeks ago playing basketball (bumped knees with someone else) and its dislocated 6 times since the initial incident (4 of those were on the way to the ER) They had me put on a knee immobilizer to keep it straight and its been about 2 weeks now. There were no bone fractures from the xrays but ortho was adamant on not giving me an MRI (Not sure why). They are trying conservative treatment first before deciding on surgery. Just went to my first session of PT today and I could not really bend my knee at all because it felt stiff and locked at the kneecap (maybe because of swelling?) which really worried me. As of right now, the knee area still feels slightly unstable but feels much more stable than about a week ago. I can also walk on it with the immobilizer on with no problems. So I have a couple of questions for you guys.

1. How long is it going to take before I can get full range of motion of my knee again?
2. About how long should it take for me to rebuild my quads/hamstrings/calves back up?
3. Chances of surgery?

1 – There isn’t a perfect answer to WHEN you’ll get full ROM back. Very individualized, and you just need to work at it.
2 – Same, rebuilding strength is individual. Depends on how much strength you’ve lost, how quickly things heal, swelling goes down, etc.
3 – Surgery, hard to know at this point. I believe the statistics on having a recurrent dislocation is 50-50 (and I personally would define the multiple dislocations you’ve already had as a general 1st time dislocation – others may disagree). And even if it becomes a chronic problem, doesn’t ensure surgery. You would need to make some decisions based on your tolerance of the instability, what you want your activity level to be, etc.

Overall, be patient with the rehab – this type of knee injury isn’t a black and white one where there is a clear course of action. So, it’s hard to answer your questions like you maybe want. Conservative treatment is the way to go with a first time dislocation, MRI’s aren’t necessary right off the bat (don’t really show much in regards to this injury) Let it heal up, get your ROM & strength back, and see where it's at.

Like Allyd said it is different for most people.  It would take me anywhere from 8-12 weeks to feel normal after a dislocation.  I had 7-8 dislocations over a period of 15 years before I had surgery and I am still recovering from the surgery.  My progress seems slower than others on here, but again it is different for each individual.  Go to Pt, do what they say to do and see what happens from there.  Good luck!

I wonder if you tore something. I also wonder why no MRI. I remember 10 years ago during a rainy day I was running up the steps of the Italian Cultural Center for a class, got to the top, slipped and fell face first down a long flight of marble stairs (I curse the Italian design aesthetic  :)) My knee was stiff, swollen and painful for a week until I went to a general OS who immediately did an MRI/x-rays and told me my grooves were shallow and that my kneecaps sat to the side and that I should never do squats and avoid stairs as much as possible and wear a brace when I exercised. All good advice. But my knee remained a tiny bit swollen forever after that and it did hurt if I didn't wear my brace when I exercised or after a long day at work when I walked a lot.
My kneecap also dislocated on its own a few times before my accident.
Your locking I suppose could be because of the swelling but I doubt it. My kneecap locked because after my accident I had a hole in my patella which was filled with bone fragment, debris and scar tissue. I'd at least want every possible test done to rule out what can be ruled out. I also wonder how they can decide on surgery without doing x-rays and MRIs first? My surgeon pored over every MRI and MRI report I had (3)
Accident 2011: permanent dislocation, hole under the patella, bone fragments/debris embedded, scar tissue, 3 torn ligaments, fracture, cartilage sheared off, contracture

Yeah, what allyd said. Just work through the rehab and see where you are after that. If you did tear something, you still may be good to go without surgery. Always to conservative first and don't even worry about the what if until you get there.
