fat pad removal

Hi
Last April, I had a TPF after been hit by a car, and ever since have had continuing pain. In November last year they removed the hardware in case that was the cause of the pain. It didn't make any difference. I'm pretty much at my wits end with the pain. When ever I bend weight bearing it hurts a lot! because of this I can't do squats, having trouble going up and down stairs/hills and my thigh muscles are extremely weak. I've had xrays and an MRI which show the fracture has healed and the cartilage looks good. I've also tried Celebrex and Lyrica with no results. My surgeon referred me to another surgeon for a second opinion. Today I saw that surgeon and he said after looking at everything, he believes there is scarring in the fat pad either caused by the damage resulting from the impact of the car hitting me, or from the surgeries. So he has suggested that I try getting it removed. I see my normal surgeon in a few weeks to talk about it and booking in for surgery. Has anyone else had their fat pad removed or can direct me where to look for information? What is the recovery time? What is the pain like?
I'm willing to try anything at the moment to get rid of this pain.

Hi Jasmin

I would suggest you try noninvasive methods first, before resorting to surgery. You can try physical therapy, ultra sound, ice massage, iotophoresis, cortisone shots, and either kinesio taping or McConnell taping. When you have exhausted those methods, then get another opinion before having surgery. Stay away from squats and anything that irritates the knee causing more inflammation because the fat pad is highly innervated. Here is a link to KNEEguru's info on the fat pad:

You will get a different answer from those who had surgery as to whether it was successful or not and only caused more pain. In my case, I had my fat pad removed because of pain and impingement. I tried all the above noninvasive methods first, before resorting to surgery. I had scar tissue removed and my doctor said the fat pad was hard and fibrotic. I also had an extensive chondroplasty to the backside of the patella. The surgery corrected the impingement and pain I had in that respect but I had a very large patella lesion, so I was still having a lot of pain from it, but was glad I had it removed.

There are some who posted replies who had surgery and wished they never had it. I would look thru all the pages of this section for other fat pad stories and also put fat pad, or hoffa's fat pad in the search box feature of this website to find other discussions.

I've been doing normal physio since May last year. Have also tried rehab clinical pilates, reformer pilates, accupuncture, TEMS machines, taping and knee brace but nothing has helped the pain.
I have read about fat pad abalation through alcohol injection which may be something else to try.
