Arthrosurface Hemi Cap - Local Orthopedic Surgeons
« on: May 18, 2011, 04:14:26 PM »
Hi,
I had a Microfracture Chondroplasty in the Femoral Condyle 6-months ago and I'm still on crutches with knee and ankle swelling. It might be time to look at other options.
Anybody please reply with local Orthopedic Surgeons in the USA - New Jersey, New York, Pennsylvania that are experienced with the Hemi Cap operation.
Thank you.

How old are you?  Do you have any other damage to meniscus or tibia? If you are relatively young and everything else is in pretty good shape I would look at other biologic resurfacing options (OATS or ACI) before putting metal in the knee.  There's a handful of very good surgeons that specialize in cartilage procedures and try to avoid putting metal in the knee as long as possible.

Dear OrthoRep,
Thanks. I'm 62 and probably too old for OATS or ACI.
My post-op MRI reports medial tibial plateau mild edema.
I'm puzzled since my reason for seeing an OS in the first place was for pain in the right knee front medial side about an 1.5 inches below the joint line when ascending / decending stairs. I also get pain there if I slightly twist my extended knee to the outside. I can also reverse an intermittant slight catch if I rub on the MCL.
 * But, the meniscectomy and microfracture (femoral condyle) were on the posterior medial side. *
I'm thinking there's another problem ... maybe a tear in the front medial meniscus or ruptured / torn anserine tendons or the mild edema in the medial tibial plateau.
Please, any ideas?

Markld,
Thanks.
Not counting the OS that did the operation, I've seen 3 additional OS's who reviewed all my reports and films.
1st one gave me a shot (ineffective), the 2nd one tried to schedule me for a PKR and the 3rd one prescribed an unloader knee brace and PT.
After 4 PT sessions, I was getting nowhere and scheduled my first session with the hospital PT yesterday. THAT'S THE ONLY WAY TO GO.
The therapist could have easily past for a doctor and concluded that my hamstring and quad muscles were non-responsive due to non-use and trauma of my operation ... 6-months ago!!!  Consequently, the three pas Anerinus tendons have severe tendonitis. During my initial 2.5 hour session, he removed the swelling from my calf and ankle with massage, stimulated the hamstrings and quad muscles electronicall, corrected my straight leg walk and gave me exercise homework.
THIS MORNING I CAN WALK (a very short distance) WITHOUT CRUTCHES AND MINIMAL PAIN! Hopefully, I'm finally on the right road to reaching a "normal" knee.
BTW, the operating OS said he was not "a believer" in PT and I'm sorry to say that I wish he be cripple and in pain like me for the last 6-months!
TO MY FRIENDS AT KNEE GURU, the right PT is a MUST even after a simple arthroscope (no surgery).

I have a PT background and agree with you 100%, however, you don't always need to go to hospital PT for good treatment.  There are good and bad in both private and hospital setting.  A good therapist will make all the difference though.  Glad to here you are on the right track now.
