Flat vs Shallow Groove

Hello all,

I am pending having my 2nd Fulkerson for patella baja-infera and congenital tracking issues that have failed to respond to many other  procedures. Long story...but now I have something new to worry about on top of all the other things. I'm a  high risk patient due to my fibrosis issues. Getting worse after the procedure is very real and it's why I've put this surgery off for so long.

It seems, I have a flattened groove. One OS called it flat (very experienced) and another young OS mentioned the word, shallow. Is there a difference? Is flattened the same as shallow. I just read another post that said a shallow groove was a negative for a person having a TTT. How about "flattened" same thing or not?

Oddly I've been seeing OS's for years and years and I only remember hearing about my flat and shallow groove recently. If this is dumb forgive me. I was wondering if my horrible patella position and the years of maltracking and pressure from the baja could have reshaped the groove I was born with.

Hi there,

I think it is one and the same thing. Just different words.

You were probably born with shallow grooves. They know much more now than they did.

I have had a trochleoplasty on my left knee, which has made a massive difference. Am waiting to have my right knee done in just under 2 weeks.

Would be getting another opinion from an experienced PFJ specialist who is in the forefront of their field. Whereabouts are you based ?

Hi Callie

Its called Trochlear Dysplasia, my right knee has a shallow groove and my left knee has a flat groove, I've had TTTs on both knees and they have both been successful but I do not have any problems with fibrosis, all I can tell you is that my grooves are pretty non-existent but the TTTs still worked for me, I had them done 10 and 15 years ago and they're still holding up!  Good luck if you decide to go ahead with the surgery.

Hi Callie,

Unless the trochlear dysplasia is treated properly you are likely to continue having problems. You are right, I don't think TTTs are appropriate if you have shallow grooves.

Now that my left knee has been addressed I don't have he same problems. I am having my right knee done on Thursday. Will be good to get it out of the way. The trochleoplasty on my left knee has made such a difference.

I would seek out the opinion of someone that specialises in patella problems in the top of their field. There are a few OS's in the US that do trochleoplasties. Whereabouts are you located ?
