Fat Pad Issues Anyone?

I was told today that I'm having some fat pad inflamation/irritation.  I'm 11 weeks post-op on a TTT and have recently started doing more extension than usual since my quad has finally started to work again.

My question is, has anyone had fat pad issues and how have they dealt with it?  I'm getting a really really sharp pain when I do very specific activities, such as extension, and when pressing on what I was told is where the fat pad is.  I'm hoping that this goes away, and I'd appreciate any advice on the topic!

Hi There,

I was told my fat pad inflammed and surrounded by fluid when I had an MRI scan done in July 2008 but been told its not causing me the problems I am getting. My right knee is swollen at the bottom and get sharp intense pain at time but most of the time its just a dull ache but know somnething wrong with the knee. It happened after a trip onto pavement in October 2007 and not been able to run since.

Had Ultra sound but did not help and an cortisone injection into the lateral side of the knee but not really helped and still get sharp pain and catching inside the knee. I am having a scope on right knee anytime now once letter arrives in the post.

I know what its like getting sharp pain and feels like a loose cartilage but an inflammed fat pad can cause the same symptoms and can be very painful. Might not be the fat pad causing me probems once my OS looks inside my knee.

The last resort would be to remove part of the fat pad or all of it but its not a good thing to have done and best avoided.

Hey Slugger,

I've had fat pad problems since my ACL recon in 2000.  It is a very frustrating condition, as you don't want to straighten your leg because of the nipping pain.  And  it's nearly impossible to strengthen your leg when you can't fully straighten it.  Then, having a weak quad causes other problems.

It's good that you are catching the problem fairly early in the process.  If you don't, your brain will learn to keep your leg from straightening to avoid the pain.  That causes the soft tissues in the back of your knee to contract a bit.   At that point, you have to retrain your knee and your brain! 

I am currently having another course of iontophoresis because my fat pad flared up this summer.  Ask your PT about it.  I wear a four-hour patch that uses a battery to deliver the cortisone.   The patch is placed right over the part that is sore.  Even though I had much of my fat pad removed in my last LOA, the little bit of pad that did remain has STILL grown scar tissue.  So, every once in a while, it flares up.  But the cortisone delivered by ionto calms it - usually about 8-10 treatments.

I was woundering why my OS did not give me more cortisone injections as I only had one so far and having my knee scoped in 9 days time and thinking maybe it not needed after all and just need a few more injections.

Hi Nick,

I am guessing that your doc also decided against it because the first one didn't work.  In addition, some studies suggest that mulitple injections may soften tendon and cartilage tissue.  And, with any injection, there is a chance of infection, albeit, a small chance.  All these factors must be weighed for each individual case.  In yours, I am guessing that since you weren't getting any improvement with the injection, there was no point in taking additional risks.
