ACI on both knees
« on: May 31, 2009, 08:59:21 PM »
I have just come out of hospital today after having my second ACI this one was for a defect on my patella on my left knee some 8 months after having same treatment on my right knee.

At first my surgeon was un willing to operate on my left knee because he feared it could damage the graft on my other knee in  the recovery stafge but eventually he had no choice.

Was wondering if any one has had this procedure on boths knees within a short time of each other and wether you ended up going in circles or you got better and returned to activities. Im worried because i was a very active athlete 14 months ago and i cant seem to find any one in my position to ask how they did and weather they got fit again

My previous op history is
Left knee arthroscopy and menisectomy age 13
righ knee arthroscopy and chondrosplasty age 16
right knee arthroscopy and biopsy for ACI age 16
right knee ACI age 16
left knee arthroscopy, chondroplasty and laterall menisectomy age 16
left knee ACI age 17
Right knee

wow elliotc92..........you are only 17 and they did both knees? What kind of activities did you do prior to your injuries? Do you think it's sports related? Did you get second opinion?

It is challenging but doable. I had my most recent knee ACI/AMZ about 18 months after the first. Both my lesions are patella. I would say 8 months would have been a little soon for me to attempt number 2. There is a lot of one legged squatting and stairs that have to be done. The one thing I would suggest is doing a ton of one-legged exercises while you are in the CPM and on crutches. Strength will be your biggest factor in success. You also might want to take a little extra time to wean from crutches. Good luck.

I did alot of football and i played as a goal keeper quite alot so that inolved alot of pounding my knees, i also did badminton cricket and boxing. My OS said it could also be congenital or just bad luck, he is reputed to be the best in the UK so he knows his stuff.
As far as the second opinion goes he has explored so many avenues before doin it again that i dont believe it is nessacery, he spent roughly 6 months trying non surgical techniques because ACI on a 17 yo is rare and he wanted me to try and get better my self. He also said that with my previous history the left knee was no chance getting better and when he scoped it it demonstrated damage down to the bone and so needed to be done any way. I didnt really have second opinion time aswell due to the fact i could potentially damage my graft on my right knee graft.

Thanks

Hi Jonhark.......seems like you know quite a bit about this procedure ...i totally agree with you in that strength is the biggest factor in overall success....how are you doing now as far activity level....are you able to jog or squat into a catcher's position with minimal pain? Has your OS mentioned stem cell procedure? Thanks for the info

2 year post op knee--yes, able to squat into catcher's position and jog

My most recent knee is not ready for squatting yet. Patella defects are a tough location but do get better over the course of 18-24 months.

I'm a little shy of six months and am able to walk 2-3 miles and do stairs mostly pain-free.

It is a long process for sure...and easy to screw up if you are over-zealous in the beginning stages.

Another question is at 8 months is it normal to still have a bit of grating and minor pain sometimes only on rare occasions or should i get it checked out. My os warned me a few months ago that he may need to go and clear out the scar tissue for me to get the bennefit but i doubt he would do that now

Jon,

That is great the way you have been patient with recovery. At what point in the recovery will you be able to get on elliptical? Which exercise do you think helped you the most? Did your OS suggest you take supplements (nutritional)?

Jon,

One other question.....did you ask your os about the stem cell procedure on your second knee? Since your lesion was not that large it might have been easier? Only 1 doctor in Colorado is doing the stem cell?

I am new to this site and I've been told that both my knee caps are shot.  I guess the years of soccer and running finely caught up with me.  I was scoped 28 years ago (age 19) on both knees and have not had problems until 6 months ago.  I am scheduled for surgery on the left knee next week. (June 29th). OS said I was a good canidate for the Carticel procedure because of my age and activity level.  From what I have read, it sounds like I'm in for a long recovery.  I just hope its all worth it.

Hi Elliot92,

I had aci done on both knees of the trochlea and only waited 5 months to do the second one. I am now 26 weeks post op on one knee and 6 weeks post op on the other.
So far I think it is workong for the one that is 26 weeks post op. My symptoms seem to be improving.
It is a very long road especially when you have 2 knees done. I am 35 years of age, so you will heal a bit faster.

April 2008 microfracture left knee trochlea
Jan 2009 ACI harvest from right knee
Mar 2009 ACI right knee trochlea
July 2009 Aci left knee
Nov 2009 left knee scope to check graft
